ANS: It is an FDA registration Renewal Period that allows all previously registered facility to renew their registration before it expires to avoid registration cancellation.Renewal Period starts from October 1, 12:01AM through December 31, 11:59 PM of every even year.
8. What is Duns & Bradstreet
ANS: A D-U-N-S® Number is a 9-digit unique identifier for businesses. Live Business Identity, which is derived from the Dun & Bradstreet Data Cloud and aggregates all the information provided for any given entity within the Data Cloud, starts with the Dun & Bradstreet D-U-N-S® Number. A D-U-N-S® Number is assigned once our patented identity resolution process identifies a company as being unique and distinct from any other in the Data Cloud.
9. How to Get a D-U-N-S Number
a) Choose the option that best fits your company.
b) Fill out the information requested. Based on your choice, you may need to provide the following information:
The legal name of your business
Your business address (Note: if you have multiple locations, apply for a separate D-U-N-S Number for each location)
Business phone number
The name of the business owner, president, or CEO
The legal structure of the business
The year the business was created
The primary business industry
The total number of full- and part-time employees
c) During the checkout process, you will be asked whether you want to expedite your D-U-N-S number request. Normal processing can take up to 30 business days. If you purchase an expedited* D-U-N-S Number you can get your D-U-N-S number within eight business days.
d) You may be contacted by a representative of Dun & Bradstreet to validate your information. Once your information has been authenticated, the process is complete. You will receive your D-U-N-S Number in an email from Dun & Bradstreet
10. What documents do I need to request a new D-U-N-S Number?
A new DUNS Number can be easily requested through our short form. The form requires that you enter in basic
company information and electronically upload two (2) documents that support the business information you
entered. We recommend you gather the documents, in the correct format, prior to beginning the request.
Document requirements are simple:
- They must contain the Legal Business Name and Current Physical Address of the business.
o REMINDER: if you are a Sole Proprietorship, your legal business name is your full legal given
name (ie. Jane A. Doe) so the document must contain your full legal name.
- Acceptable formats: .gif, .jpg, .jpeg, .png, .doc, .docx, .xls, .xlsx, .csv, .txt, .zip
- Do Not Include personal information such as Driver's License, Passport, Social Security Number,
Banking Statements, etc.
